By sending your comments to Palm, Inc., you agree to the following Publicity Authorization.

Palm, Inc. and/or its subsidiaries worldwide are considering using information about the experiences the Customer named above (Customer) has had with handhelds and/or smartphones from Palm for publicity purposes to be published by Palm, Inc., for distribution worldwide to press and analysts. This information may be published and distributed in any number of forms, including but not limited to print media, electronic media, audio/visual media, and/or via the Internet.

Customer hereby authorizes Palm, Inc., to use the submitted story for the purpose(s) described above. Customer agrees and acknowledges that Palm, Inc., will not be required to seek further approval from Customer in connection with its publication of such information for such purposes. Should Palm, Inc., wish to use the above information for any other purpose, Palm, Inc., agrees that it will provide the designated Customer representative with advance copies of any such information referencing Customer for approval prior to publication. Customer will be deemed to have approved any such information if it has not provided Palm, Inc., with comments within three (3) business days following submission to Customer of such advance copy. Consent to such publication and distribution will not be unreasonably withheld by Customer.

This Publicity Authorization is intended as the complete agreement on this subject matter between Palm, Inc., and Customer. This Publicity Authorization will not modify any obligations Palm, Inc., may have to Customer pursuant to any Nondisclosure Agreement or other agreement that may be in effect between the parties. This Publicity Agreement will be governed under the laws of the State of California, excluding its conflict of laws rules. The parties have caused this Publicity Authorization to be executed by their duly authorized representatives on the date shown. Submission of this form constitutes Customer's agreement with all of its provisions.
